Does this handle only come with what is shown? Does it include the water temperature plate that connects on top of this (with the hot and cold indicator)? If not, this that piece available for sale without buying a whole new unit?

This replacement volume handle does not include the temperature knob on top. The temperature knob and cover are available as RP75440. See the parts diagram for more information:
